The clinic is open to all residents of Kosciusko County for immunizations every Monday, Wednesday and Friday from 8:30 am until noon and reopens at 1:15 pm to 3:30 pm. On the first Monday of every month there is an evening immunization clinic from 6 pm until 8 pm. The Health Department Clinic is located at the K21 Health Services Pavilion, 1515 Provident Drive, Suite 160, Warsaw, Indiana. The H1N1 shot clinic is provided free of charge by the Health Department. Please note: H1N1 Vaccinations are also available through local pharmacies and some Doctor's offices.
